The Additional/One Time Pay Form has been replaced with an electronic version on the Department Administration homepage.

If the One Time Pay is an initial faculty appointment, it will require all appropriate backups for a new employee plus the Electronic Additional/One Time Pay Form. The job aid can be found here.

If the Additional/ One Time Pay is a re-appointment on an existing one time pay, only the Electronic Additional/One Time Pay Form is to be completed.

If a faculty member is already employed at 1.00 FTE and the Additional/One Time Pay is being paid in a secondary department, a dual compensation form must accompany the Additional/One Time Pay Form.
