Additional/One Time Pay Instructions

If the one time pay is an initial faculty appointment, it will require all appropriate backups for a new employee plus the Additional/One Time Pay Form.

If the Additional/One Time Pay is a re-appointment on an existing one time pay, only the Additional/One Time Pay Form is to be completed.

If a faculty member is already employed at 1.00 FTE and the Additional/One Time Pay is being paid in a secondary department, a dual compensation form must accompany the Additional/One Time Pay Form.

Forward the original Additional/One Time Pay Form with all necessary backups, from the office of the academic dean to the Office of Faculty Development and Advancement. 

Note: If paid through C&G funds, send the original from the office of the academic dean to SRAS and SRAS will forward to the Office of Faculty Development and Advancement.
